ULTIMATE NURSE PRACTITIONER OPPORTUNITY San Antonio, Texas Reliant Post:Acute Care Solutions is looking for a NP/PA to join our growing and dynamic team of post:acute providers at one of our facilities in the SAN ANTONIO area. Our division, Reliant Medical Management (RMM), is a medical practice management company specializing in the management of post:acute/skilled nursing facilities (SNF). Our advanced practice providers are highly trained in the specialized services and regulations that are unique to the geriatric population and to long:term care. Using best practice guidelines and clinical research to formulate clinical protocols, in addition to incorporating rehabilitation and nursing services together, gives our patients the best possible outcomes. We are looking for motivated and talented advanced practice providers to join our team Must be able to work independently. Business hours are Monday:Friday (days) with no week:ends required Reports to: VP of Clinical Services Job description: Under the general supervision of the VP of Clinical Operations, plan, direct, and evaluate resident medical care. Examine and treat chronic and acute episodic illnesses, minor injuries, accidents, and other injuries/illnesses. Perform selected laboratory testing, referrals to physicians, as needed, write prescriptions, and perform procedures within the scope of expertise, protocols and available equipment. Work in collaboration with the facilitys medical director to provide care and treatment services for all residents. Essential Duties and Responsibilities: Provide exceptional customer service to residents, families and/or responsible parties, facility staff and attending physicians. Maintain an appointment calendar. Evaluate new residents upon admission to the facility, if not initially seen by attending physician. Assess residents prior to discharge from facility to home or Assisted Living Facility (ALF) Attend to residents in the facility for selected physical examination, diagnosis and treatment of both chronic diseases and acute episodic illnesses and injuries (as evidenced by Quality Indicators) Practice medicine aligned with evidence:based medical recommendations, regulatory requirements and formularies. Perform procedures within the scope of expertise, protocols and available equipment Schedule follow:up visits, as needed, and aligned with AMDA White Paper on Medical Necessity Guidelines and Federal Guidelines. Confer with residents attending and consulting physicians, as needed. Confer with residents and their family and/or responsible parties regarding the residents status Complete appropriate Medicare Part D Prior Authorizations in a timely manner. Attend clinical portion of facilitys daily morning meeting. Review statistics and conduct research, as deemed necessary for the benefit of the facility. Compile necessary reports. Collaborate with facility staff to identify residents medical needs and help develop an appropriate plan of care. Take after hour calls per agreement Provide daily activity logs to the billing department. Obtain and provide accurate and complete financial information for residents upon initial encounter Attend required conference calls, webinars, etc. Other special projects and duties, as assigned. Job Requirements: Masters degree in Nursing (MSN) required for Nurse Practit
